The Service Coordinator plays a key role in connecting residents with community-based programs, services, and resources through a person-centered, non-clinical approach. Working under the supervision of the Property Manager or Program Manager, the position focuses on coordinating both group-based activities and individualized services that address social determinants of health, wellness, and independence.
Responsibilities include building relationships with local service providers, organizing educational and wellness events, coordinating health screenings, and assisting residents with access to services such as transportation, meals, housekeeping, financial assistance, and counseling. The Service Coordinator also conducts assessments, develops care plans, documents services, and monitors ongoing support to ensure residents’ needs are being met in accordance with federal, state, and program requirements, including HUD guidelines. The role requires a high school diploma or GED, with a bachelor’s degree in social work or a related field preferred. Candidates should have two or more years of experience in social services, particularly working with older adults, individuals with disabilities, or families. Strong communication, organizational, and advocacy skills are essential, along with basic computer proficiency in Microsoft Office and web-based systems.
